Title: EFFICACY OF UTERINE ARTERY EMBOLIZATION IN THE MANAGEMENT OF UTERINE ADENOMYOSIS

Background: Uterine adenomyosis is a benign gynecological condition where endometrial tissue grows within the uterine myometrium, leading to symptoms such as menorrhagia, pelvic pain, and infertility. Among these, menorrhagia is the most common and debilitating symptom in patients with uterine adenomyosis. While traditional treatments like hormonal therapy and hysterectomy are frequently used, uterine artery embolization has emerged as a promising minimally invasive alternative. Recent studies suggest that UAE is effective in reducing menorrhagia and improving quality of life in these patients (Sotoudeh et al., 2022; Van der Kooij et al., 2023). Objective: This study aims to evaluate the efficacy of UAE in the treatment of uterine adenomyosis, focusing on the symptom relief of menorrhagia, reduction in uterine volume, and improvement in quality of life. Methods: A retrospective analysis was conducted on 20 patients diagnosed with uterine adenomyosis who underwent UAE between June 2023 ? December 2024. Menorrhagia was assessed clinically through patient-reported menstrual blood loss, including frequency, duration, and volume of bleeding. Pain levels were evaluated using the Visual Analog Scale (VAS). Pre-procedural assessments also included imaging with MRI and USG and quality of life questionnaires. Follow-up evaluations at 1, 3 and 6 months included assessment of clinical improvement in bleeding patterns, uterine volume reduction, pain scores, and overall quality of life (Gottfried et al., 2021). Results: Significant improvement in menorrhagia was observed, with 95 % of patients reporting a reduction in heavy menstrual bleeding after UAE. Pain scores also showed significant improvement, with a 75 % reduction in pelvic pain. Uterine volume decreased by 70 % on average, and 92 % of patients reported an overall improvement in quality-of-life post-procedure.Minor complications, including mild post-embolization syndrome, were noted in 5 % of patients,with no major adverse events (Chevallier et al., 2023). Conclusion: Uterine artery embolization is an effective and safe treatment for uterine adenomyosis, particularly for patients with menorrhagia. UAE provides significant symptom relief, including reduction in heavy menstrual bleeding, uterine volume reduction, and improvement in quality of life. This minimally invasive procedure offers a valuable alternative to more invasive treatments such as hysterectomy, particularly in women seeking fertility preservation or those who are not candidates for traditional interventions (Huang et al., 2023).
