A Case Report on Right Ruptured Ovarian Ectopic Pregnancy Introduction: Ovarian ectopic pregnancy (OEP) is a rare but potentially life-threatening condition, accounting for 0.5-3% of all ectopic pregnancies. It occurs when a fertilized egg implants in the ovary instead of the uterus. If untreated, rupture can lead to severe hemorrhage and is a major cause of maternal morbidity and mortality. Materials and Methodology: A 24-year-old woman (G2P1L1) presented with 6 weeks of amenorrhea, acute pelvic pain, and vaginal bleeding for the past 4?5 days. She was clinically stable but had localized tenderness in the lower abdomen. Transvaginal ultrasound suggested an adnexal mass with free fluid, consistent with a ruptured ovarian ectopic pregnancy. Given the diagnosis, an exploratory laparotomy was planned. Results: During surgery, a ruptured ovarian ectopic pregnancy was found in the right ovary with significant hemorrhage into the peritoneal cavity. The right ovary was removed via right oophorectomy. The patient recovered without complications and was discharged on postoperative day 2 in stable condition. Conclusion: Ovarian ectopic pregnancy, though rare, can be life-threatening. Early diagnosis with imaging and prompt surgical intervention are key to reducing morbidity and mortality. In cases of rupture, oophorectomy is often necessary, and while laparoscopic surgery is preferred when feasible, exploratory laparotomy remains a safe and effective approach for complex or unstable cases.
