Abstract :
Lymphoplasmocytic lymphoma (LPL) is a B cell malignancy involving bone marrow, lymph nodes, and spleen. Its subtype, Waldenstr?m macroglobulinemia, is marked by IgM monoclonal paraprotein in blood.Lymphoplasmocytic lymphoma is an indolent cancer having an incidence of 3-7 per million per year and a male predominance,usually presenting in the seventh decade of life,is a very rare condition and only a few cases are reported from India. A 28-year-old woman, G4A3, at 36+4 weeks with stage 4c lymphoplasmacytic lymphoma with WHO Class 3 cardiac risk , and positive anticytoplasmic antibodies came for safe confinement of pregnancy.Her management required a multidisciplinary approach due to cardiac condition, fetal growth restriction(FGR) and altered haematological parameters.Prophylactic vacuum-assisted delivery was performed , and a healthy baby was delivered .The patient experienced atonic postpartum hemorrhage which was appropriately managed and haemostasis achieved. Contrary to the available data,our patient was a young, reproductive age group female which is a sporadic case.Due to regular antenatal visits and follow ups the patient was admitted at the appropriate time and delivery was planned.Prompt decisions were taken and undelayed management of PPH during the peripartum period rendered the patient stable . The patient was on continuous intense monitoring by a multi speciality team as she had increased risks for thromboembolic and hemorrhagic events. The patient was advised for regular follow ups and successfully discharged. This case highlighted the necessity of periodic imaging and haematological workup which was very crucial in assessing the outcome and for managing the complications that occurred during pregnancy and in the peripartum period.Personalised interdisciplinary care, rapid intervention and management was the key for a successful pregnancy outcome in this patient .
