Author: Dr Sasirekha L Co-author: Dr Vijayasheela P INTRODUCTION : Fibroids are benign, monoclonal tumors of smooth muscle of myometrium.As fibroid enlarge, they may outgrow their blood supply resulting in cell death. Types include hyaline, cystic,hemorrhagic, fatty, calcification. CASE:A 35 year old nulliparous woman,married since 1 month, came to outpatient department with complaint of mass per abdomen,dyspepsia,difficulty in breathing, irregular menstrual cycles since 1 year.Patient is stable and general condition is fair.Per abdomen findings:A mass of 28 weeks size,cystic in consistency, restricted mobility, regular margin, smooth surface, all borders felt except lower border.Per speculum findings: cervix and vagina appears healthy, cervix drawn upwards , uterus size couldn't be made, cystic mass felt highup in anterior and both- lateral fornices.MRI and USG findings sh 1owed large complex cystic lesion of size 25x20x17 cms favouring ovarian neoplasm. Patient was posted for staging laparotomy after taking informed consent. Intra operative findings showed large complex cystic mass of size 18x15x12 cms arising from uterus and left ovary adherent to the mass.Total abdominal Hysterectomy with left salphingo oopherectomy done. HPE findings revealed degenerative fibroidith cystic changes. CONCLUSION:Although it is rare in this age group, it is important to evaluate the every complex cystic mass for degenerated (cystic) fibroid and prevent the complications
