Abstract :
Introduction: Thecomas, a relatively common benign type of sex chord stromal tumors, most common in post menopausal age (mid 60?s), infrequently before 30 yrs of age. It can be associated with granulosa cell tumors. Thecomas produce excess estrogen, due to which patient may have abnormal vaginal bleeding, pelvic mass or both, Endometrial hyperplasia, adenocarcinoma. Thecomas are the solid tumors that resemble the theca cells. Because of this texture these tumors appears as solid adnexal masses. Bilateral involvement is rare. But ovarian Thecomas are clinically benign and surgical resection is curative. Case report: A 64 yr old, P2L2 tubectomised with post menopausal status with k/c/o hypertension with k/c/o hypothyroidism came with complaints of increased per vaginal bleeding since 1 month. Menopause attained at 46 yrs of age. On per abdomen examination, mass corresponding to 18 weeks size, irregular in shape. Per speculum ? bleeding through cervical os noted. Per vagina- uterus ? 18 weeks size, Rt. Forniceal fullness present ; left fornix free and non tender. Scan finding showed well defined adnexal mass of 10?7 cm noted- likely right ovarian origin . patient underwent exploratory laparotomy with frozen section and proceed. Frozen section showed few signs of thecoma, following which total abdominal hysterectomy with bilateral salpingo-oopherectomy was done. Hpe sample was sent and it showed signs of sex chord stromal tumors ? likely thecomas
