Ovarian mucinous cystadenomas are common benign tumours but can grow to large sizes and have the potential to evolve into malignant or borderline tumours. We report a rare case of a giant ovarian mucinous cystadenoma in a 57-year-old post-hysterectomy woman. The patient presented with abdominal bloating, constipation, and increasing abdominal girth over 6 months, with associated pain. Imaging revealed a large, multiloculated cystic mass, and tumour markers were within normal limits, suggesting a benign neoplasm. Given the tumour?s size and the patient?s postmenopausal status, a surgical approach was adopted, including exploratory laparotomy with left oophorectomy. Intraoperative frozen section biopsy confirmed the diagnosis of benign ovarian mucinous cystadenoma. The patient recovered well postoperatively. This case highlights the importance of continued surveillance for ovarian pathology even in post-hysterectomy women, as residual ovarian tissue may still develop benign or malignant tumours. Careful evaluation and surgical intervention remain crucial in managing such rare presentations.
