Introduction: Cellular Angiofibroma of the vulva is a rare benign mesenchymal tumor first reported in 1997. It often presents as a painless, slow-growing mass that may be confused with other fibroblastic tumors. Aims and Objectives: This case report aims to document a rare occurrence of vulvar cellular angiofibroma coexisting with a Bartholin's cyst and decubitus ulcer, detailing the clinical presentation, diagnostic challenges, and therapeutic approach. Material and Methods: A 38-year-old female presented with a pedunculated mass on the left labia majora. Diagnostic procedures included physical examination, ultrasonography, and histopathological analysis following surgical excision. Results: The excised mass was confirmed as cellular angiofibroma associated with a benign vulval cyst, likely a Bartholin's cyst. Histopathology showed fibromyxoid stroma with scattered vascular channels, and no malignancy was noted. The patient had an uneventful recovery with no recurrence at six-month follow-up. Conclusions: Vulvar cellular angiofibroma, though rare, should be considered in the differential diagnosis of vulvar masses. Surgical excision remains the treatment of choice, with histopathology providing essential diagnostic confirmation. Awareness of this condition can aid in timely, accurate diagnosis and management, preventing unnecessary interventions.
