Introduction: Obstructed Hemivagina and Ipsilateral Renal Anomaly (OHVIRA Syndrome) or Herlyn Werner Wunderlich Syndrome, is a rare congenital anomaly of defective fusion of paramesonephric (Mullerian) ducts during embryonic development. It includes a triad of uterine didelphys, obstructed hemivagina and ipsilateral renal anomaly. It frequently presents with abdominal pain, dysmenorrhea and abdominal mass secondary to hematocolpos. Case Report: We are reporting a case of a 14 year old girl presented with lower abdominal pain since 1 month. She attained menarche at 13 years of age and had regular menstrual cycles associated with dysmenorrhea which progresses in severity. No history of urinary symptoms On per abdominal examination, a palpable pelvic mass is noted with tenderness. On per vaginal examination, bulge noted in the upper part of vagina. Discussion: Further work up with Ultrasound, CT and MRI Abdomen and Pelvis demonstrated presence of 2 uterine cavities, 2 vagina and 2 cervix s/o uterine didelphys. Left hematocolpos and hematometra with low left hemivaginal obstruction was demonstrated. Left renal fossa was empty s/o left renal agenesis. On laparoscopic exploration, Right horn and left horn of uterus noted measuring 2x2 cm and 8x 6 cm respectively. A single stage vaginoplasty was done. On re-laparoscopic examination, bulge at cervicovaginal or vaginal region below left horn was reduced. Conclusion: This case highlights the importance of rare congenital anomaly known as OHVIRA Syndrome.
