Abstract :
INTRODUCTION Leiomyomas are classified as benign mesenchymal neoplasms and consist of smooth muscle cells with variable amounts of fibrous stroma .The tumor occur most frequently in the uterus, but vaginal wall leiomyomas are rare with only 300 cases reported since the first case was described in 1733. These tumors are thought to arise from Mullerian smooth muscle cells in the sub-epithelium of the vagina mass. Here we present a case of vaginal leiomyomas who presented as vaginal wall mass. HISTORY- A 44 year old female para3 living 3 with permanent sterilization came with complaint of heavy menstrual bleeding since 3 years with passage of clots. MANAGEMENT Total abdominal hysterectomy with salpingectomy done And proceeded for Bartholin cyst excision which turned outto be soft cystic mass of 6*7 cm. On cut specimen it showed whorl like appearance suggesting of leiomyoma DISCUSSION- ? Vaginal leiomyoma -usually seen in age group of 35-50 years old. ? More common among Caucasians women while Uterine leiomyoma - more common in non-caucasians groups. ? Usually these tumors are single, benign and slow growing but sarcomatous transformation has been reported. ? Preoperatively diagnosis by ultrasonography may be difficult but MRI usually clinches the diagnosis. However histopathological confirmation is the gold standard of diagnosis. ? Most effective approach to surgery depends on the size and location of leiomyoma. CONCLUSION Although rare, care must be given in identifying the vaginal tumor at an early stage in view of its malignant potential . Vaginal leiomyoma may present with various presentation. It has been advisable to execute enmass enucleation to prevent its recurrence and malignant transformation.
