Title: ANATOMICAL AND FUNCTIONAL OUTCOMES OF MCINDOE?S VAGINOPLASTY: A TERTIARY CARE HOSPITAL EXPERIENCE

Abstract :
Introduction Women with Mayer Rokitansky Kuster Hauser (MRKH) syndrome with an absent vagina and internal genitalia. These girls need neovagina creation for sexual function. Aims & Objectives To analyse the surgical and functional outcomes in women undergoing Mcindoes vaginoplasty at our institute over a period of 2 years (January 2021 to December 2023). Material & Methods A total of 30 girls presenting to gynaecology OPD with primary amenorrhea and subsequently diagnosed as MRKH were recruited prospectively after taking ethical clearance. All underwent a preoperative assessment and thorough counselling about fertility outcomes. All girls were taken up for surgery near planned marriage. The intraoperative details regarding type of mould, vaginal length pre-, post-surgery and at follow up, intraoperative and immediate post operative complications and, follow up sexual function and late complications were noted. Results Of the total 30 girls,3 were married prior to surgery and rest were unmarried. The average age was 21.8 years at presentation. 1/30 had renal anomaly and 3/30 had spinal deformities. During the procedure, both amnion and surgical were used for neovagina creation. There were no intraoperative complications. 2/30 had immediate minor post operative complications like discharge PV managed conservatively with antibiotics and 4 had restenosis post-surgery, 2 requiring redo surgery. The average preop vaginal length was 0.5 cm and average post operative vaginal length was 8 cm. At 6 month follow up the average vaginal length is 6 cm and no restenosis requiring subsequent surgery. The average FSFI score was 29.3 suggesting satisfactory sexual functions. Conclusions Mcindoes vaginoplasty is a safe and effective surgical procedure for young girls with MRKH syndrome to have chances of healthy sexual relationships.
