Abstract :
Introduction: Placenta previa (1 case per 300-400 deliveries) is one of the leading causes of third trimester haemorrhage which leads to significant maternal and perinatal complications. Aims and Objectives: To evaluate the maternal and neonatal outcomes in cases of placenta previa in both scarred and unscarred uterus. Materials and Methods: A cross sectional study was conducted among 40 antenatal mothers presenting with placenta previa (either presenting with APH or Ultrasonography confirmed) in third trimester attending the Gynaecology and Obstetrics department of Calcutta National Medical College and Hospital. Results: Among 40 cases of placenta previa, 21 had scarred and 19 had unscarred uteri. Most patients (57.5%) were aged 21?30 years. Obstetric complications like PPH (62.5%) and hysterectomy (75%) were more common in scarred uteri. Poor neonatal outcomes were higher in the scarred group but not statistically significant. ICU admissions were significantly higher in scarred uteri (70.8%) compared to unscarred uteri (29.2%) (p=0.004). Conclusion: The maternal and fetal outcome is seen to differ according to previous history of uterine surgery with significant intraoperative complications and postoperative morbidity seen to be higher in scarred uterus group. Accurate diagnosis, judicious expectant management with blood transfusion as required, and timely delivery can lead to the most favorable outcome.
