Abstract :
INTRODUCTION. Aplastic anaemia, a rare immune-mediated disorder causing pancytopenia and hypocellular bone marrow, poses significant challenges during pregnancy due to overlapping symptoms and limited treatment options. Pregnancy increases relapse risk, complicating management with restricted therapies potentially jeopardizing maternal and foetal outcomes Aims and Objectives: This abstract highlight the clinical presentation of aplastic anaemia in pregnancy, emphasizes the importance of a multidisciplinary approach for improving maternal and foetal prognosis, and evaluates outcomes in affected pregnancies. Case Report A 26-year-old Primigravida, at 28 weeks of gestation with known case of aplastic anaemia under haematologist care referred for specialised antenatal management. She was on immune suppressive therapy prior to pregnancy and was in remission when the urine pregnancy test is positive. Until 30 weeks of gestation she was on supportive care not requiring any definitive medications. At 30 weeks of gestation, there was a relapse of pancytopenia and was started on calcineurin inhibitor . Packed red blood cells (PRBC) were transfused during the course of pregnancy. After starting immune suppressive therapy her blood counts improved. All other routine investigations and scans were found to be normal. Trial of labour was given and Caesarean section was done at 39 weeks 3 days for obstetric indication under general anesthesia Increased blood pressure readings were noted during the postpartum . Neonatal outcome was uneventful with no need for NICU admission. Patient was followed postnatally in close liaison with haematologist Conclusion: Aplastic anaemia in pregnancy requires close monitoring and timely interventions . A comprehensive understanding and knowledge about indications and side effects of immunosuppressives on maternal and foetal health is essential for all the obstetricians and haematologists for better outcomes.
