Title: ROLE OF POSTOPERATIVE HORMONAL TREATMENT IN PREVENTION OF RECURRENCE OF ENDOMETRIOSIS IN ADOLESCENT ? A SECONDARY ANALYSIS

Abstract :
Introduction ? Endometriosis is a progressive condition beginning in adolescence with high recurrence despite surgery. Post-operative hormonal therapy induces a hypoestrogenic state which theoretically can reduce recurrence and improve quality of life. Aim and objective - To determine the effectiveness of hormonal therapy after surgery for endometriosis to reduce recurrence of disease in adolescents. Material and methods - This analysis was conducted after performing comprehensive literature search and combining patient data from four individual studies. The studies included compared surgical treatment only to postoperative hormonal treatment in adolescents between 10 to 21 years of age reporting recurrence of endometriosis. Studies that did not report follow up data and recurrence of disease were excluded. Recurrence of disease and follow up duration was collected and analysed. 71 patients received surgery only while 143 patients received postoperative hormonal therapy were analysed. Results - Recurrence of disease was reported in 25 patients (35.21%) out of 71 patients managed with surgical treatment only as compared to 40 patients (27.97%) out of 143 patients who received surgery followed by hormonal treatment (p=0.454). Despite having more percentage of early-stage disease patients in surgery only group, there was overall a higher proportion of recurrence rate in surgery only group indicating trend of benefit with postoperative hormonal therapy but did not demonstrate statistical difference in reduction of endometriosis recurrence. Conclusion ? There was no statistical difference found in the reduction of endometriosis recurrence with use of postoperative hormonal therapy in adolescents. Due to retrospective design of studies available for analysis, there is an urgent need to conduct more robust studies. There is an immediate need for creating databases for long-term follow-up of adolescent with endometriosis to determine the most effective treatment to prevent recurrence of disease with associated improvement in quality of life.
