Title: RUDIMENTARY HORN ECTOPIC PREGNANCY IN A UNICORNUATE UTERUS: A RARE CASE REPORT

INTRODUCTION: Pregnancy occurring in rudimentary horn of unicornuate uterus is presumed to occur by transperitoneal sperm migration through the normal half of the uterus and tube to the tube connected to the rudimentary horn. AIM: To present a rare case of ruptured rudimentary horn ectopic pregnancy which presented in labor room of MKCG MCH, Berhampur, Odisha. This poster shows the findings, diagnosis, and management of the above case. CASE REPORT: A 21year lady presented with complaints of severe abdominal pain, weakness and dizziness. On examination, patient?s abdomen was distended. After doing a transabdominal ultrasound, a diagnosis of ruptured ectopic was made and patient was posted for emergency laparotomy. Around 1500 ml clotted and unclotted blood suctioned out. An embryo with gestational sac present attached to Right Ruptured rudimentary horn of unicornuate uterus. Rudimentary horn resection done with ipsilateral salpingectomy. RESULT: The wide reported range of the incidence of rudimentary horn pregnancies (1 per 75000 to 1 per 150, 0000 pregnancies). This usually reflects the rarity of the condition. Early diagnosis can be life-saving. The rupture occurs because of the underdevelopment of the myometrium and a dysfunctional endometrium. Transvaginal ultrasound is the gold standard for imaging both uterine and adnexal pathology. Upon diagnosis of pregnancy in a rudimentary horn, the standard treatment consists of immediate excision of the horn and the ipsilateral fallopian tube due to the high risk of rupture. CONCLUSION: The diagnosis of a rudimentary horn pregnancy remains difficult. Due to serious maternal and fetal complications, a high index of clinical suspicion and ultrasound for uterine malformations early in the gestation can reduce the mortality rate. Most reported cases concern patients who either are symptomatic or suffer a uterine rupture with hemoperitoneum as happened with this case.
