Abstract :
Placenta accrete syndrome is one of the most feared complication during pregnancy and delivery. Incidence of this is rising as caesarean section rates are increasing and operative procedure in uterus also rising. Risk of life-threatening hemorrhage during delivery is very high so identification of high risk cases is of paramount importance. Preparedness, obstetric kill of doctor and availability of blood products holds the key component in management of such cases. In some advanced cases placenta may invade beyond the uterus involving bladder and bowel. Antenatal diagnosis by ultrasound and MRI is very crucial to manage these cases successfully. Cesarean hysterectomy without trying to remove the placenta after delivery of fetus is the treatment of choice. Still long-term morbidity is very high like injury to ureter, bladder, bowel and risk of fistulas are very high. Other treatment option like uterine artery embolization, leaving behind the placenta and observing for spontaneous absorption are available. Here we are presenting a case of placenta percreta invading the bladder which was diagnosed intraoperatively and managed successfully.
