Abstract :
Cervix is a magical organ which bears the weight of pregnancy for 9 months. But in some cases the cervix opens up prematurely which leads to a second trimester abortion or an extreme preterm delivery. So the technique to prevent such a situation is to put a cervical cerclage usually a McDonald's or a Shirodkar's cerclage. But in case of a dilated cervix or a short cervix in mid 2 nd trimester our routine cerclage might not hold up.This will require an intraperitoneal cerclage above the level of uterosacral ligament. So we in our hospital approach emergency intraperitoneal cerclage by vaginal route.
The aim of our study was to reduce prematurity and the problems associated with it.
This study was an Observational study conducted in Rajagiri Hospital.
Sample size - 32 ( 21 in dilated cervix and 12 in short cervix with funelling)
84% (28) of patients in the study group was nulliparous.42% of patients belonged to the age group of 26-30years. Study group included 7 set of twins. 57%(19) of patients delivered after 34 weeks of gestation. 3 patients expelled within few days of the procedure before crossing viability. Out of the 32 babies ( including twins ) who were born after crossing period of viability ; 71% of babies were above 2 kgs at birth.
The conclusion of the study is that by prolonging the pregnancy by atleast few weeks will improve the neonatal survival rate and reduce the problems associated with extreme prematurity to an extent.
