Ovarian Ectopic pregnancy is one of the rarest form of ectopic pregnancies in clinical practice with an incidence of 1 in 7000 to 1 in 40000 live births. We present an interesting case of a live Ovarian Ectopic pregnancy who presented to our tertiary care institute in Emergency with complains of 2 months amenorrhea and pain in abdomen with bleeding per vaginum since 4 days with past history of Tubal Ectopic pregnancy with Salphingectomy at our institute 3 years back. Examination revealed a tender adnexal mass on the same side as the previous ectopic pregnancy and USG revealed an adnexal live ectopic pregnancy not seen separately from the ovary. Exploratory Laparotomy was performed and ovarian ectopic was excised and ovarian reconstruction was performed salvaging the ovary. In conclusion early diagnosis can help salvage ovarian function and reduce maternal morbidity
