INTRODUCTION - Placenta accreta spectrum disorder?is a pregnancy-related disorder responsible for post-partum morbidity and mortality, associated with intractable or massive hemorrhage, leading to uterine loss in up to 64% of women. the main goal of placneta accreta spectrum management is to enable delivery in an expert centre in the presence of an experienced team at an appropriate time AIM -To present the management strategy of patient with placenta accreta and previa post classical cesarean section with uterine artery embolisation . MATERIALS AND METHODS-Hematological,biochemical , imaging studies were done . CASE REPORT -A 37 year old woman who is P3L2D1A2 status post classical cesarean section with uterine artery embolisation in view of PAS disorder came with complaints of bleeding per vanginum and foul smelling discharge on post operative day 64 .Through multidisciplinary approach MRI contrast imaging followed by sepsis work up was done .After effective counselling patient was taken up for hysterectomy with bladder repair .Intraoperative and postoperative period was uneventful . CONCLUSION -Management of complex PAS cases requires multidisciplinary expertise in detection ,patient counselling and surgical skill to manage safely .
