Gestational Diabetes mellitus being one of the most common metabolic derangement in a pregnant female can be a nightmare for Obstetricians when the outcome turns out to be intrauterine Fetal demise. Here is a case of 31 yr G5P3L1IUFD2MTP1 ,with previous 3 LSCS with k/c/o overt DM with history of cerebral venous sinus thrombosis with seizure disorder .Severe Dehydration due to diabetic keto acidosis led to CVST with cerebro vascular embolism 4 yrs ago in this woman causing facial palsy and hemiparesis following which she was started on Tab. Acitorm and was later shifted to Inj LMWH during here ANC period. Pregnancy being a hypercoaguble state with bad Obstetrics history with highly uncontrolled sugars HBA1C of 10.2 decision to terminate the pregnancy at 33.1 weeks gestation was taken. Delivered a male child of 2030gm kept for observation and weight gain at intensive care Unit.
