Mesonephric cyst is a vestigial remnant of the Wolffian duct in females. Its incidence is 10% in teens. Occasionally, the mesonephric duct persists lateral to the vagina and uterus, and cystic lesions may sometimes develop into neoplastic lesions. Mesonephric cysts are very rare. This is a case of a 15 year old who was apparently normal 6 months back, developed pain abdomen which aggravates during her menstrual cycle, associated with 4-5 episodes of vomiting since 5 days. On examination no palpable mass was felt, and ultrasound scan showed a well defined multiloculated cyst in the midline of umbilical region measuring 7.3x5x8.5cm. CT scan finding being large (>10cm size) multiloculated cystic lesion in pelvis. CA-125 value being 15IU. Laparoscopic left ovarian cystectomy under general anaesthesia was done. HPE report showed features suggestive of benign development cyst suggestive of mesonephric cyst.
