Abstract :
TITLE: Effect of age on lower urinary tract symptoms (LUTS) and urodynamic parameters in post-menopausal women INTRODUCTION: Lower urinary tract symptoms are common in women. It is difficult to determine the prevalence due to the multifactorial etiology, and the frequency is known to increase with age. The physiology of the bladder and urethra involves storage and voiding functions, storage dysfunction includes symptoms of frequency, urgency, nocturia, urge and stress incontinence. Voiding dysfunction may include slow stream, hesitancy, straining to void and incomplete emptying. These bothersome symptoms affect men and women of old age. Women traditionally have a higher incidence of storage symptoms such as stress urinary incontinence due to pelvic floor relaxation. AIMS AND OBJECTIVE: To assess the effect of menopausal age on LUTS of females, to perform urodynamic study, evaluate the bladder function and to correlate it with parity, hormonal therapy, BMI, associated medical and surgical disorders. MATERIAL AND METHODS: This was a prospective observational study conducted at NSCB MCH, Jabalpur. Women who visited the gynecology opd with bothersome LUTS, valid quality of life questionnaire and urodynamic study was performed on them. The various urodynamic parameters interpreted, correlated with symptoms and their medical and surgical disorders. RESULTS: A total of 20 women were enrolled and analyzed. The urodynamic tests showed the maximal cystometric bladder capacity, the average and maximum uroflow rates, the pressure of detrusor contractility at maximum urine flow rate were all significantly decreased with age. The voiding time and post-voiding residual urine volume significantly increased with age. All storage symptoms did not change with age except for nocturnal enuresis. However the effect on voiding symptoms significantly increased with age including straining to void, hesitancy and intermittency. CONCLUSIONS: Lower urinary tract symtoms and bladder functions particularly voiding functions change significantly with age in menopausal women. Urodynamic study remains a valuable diagnostic test providing information to surgeon and patient prior to invasive treatment. Author: Dr Megha Khurana Guide: Dr Bharti Sahu
